Temporomandibular ankylosis is a disease with limitation of the joint movement. Treatment depends upon the local findings and usually a gap arthroplasty with an interpostional graft is carried out. Though such varied synthetic and biological materials being used by various surgeons in the treatment of this problem, reccurrent rate is reported to be significantly higher. Frequent complications reported were, limited range of intrinsical opening due to relapse, loss of vertical height of the affected ramus, and reankylosis to name a few. In addition, foreign body reactions were reported with silicone use not withstanding the cost of the material. Extrusion and reankylosis have also been described after the use of acrylic balls and cylinders. In this report, we have experienced a case of bilateral TMJ ankylosis suspected to be caused by trauma in 30 years old man. Following the conservative treatment, the operative relief of the ankylosis was performed by an interpositional alloderm. Six months follow-up observation revealed a normal mandibular development without any reccurence. Judging from both clinical aspects and ceplalometric analysis the result is very satisfying not only functionally but aestheically. We feel that this technique is simple, cost effective and minimize donor morbidity and secondary surgeries.
