Z-plasty Achilles Tendon Lengthening

Partial and full thickness burns over the ankle and Achilles tendon are a significant source of morbidity in burn patients. In this paper, I present a simple technique that allows patients with burn scar contractures of Achilles tendon to walk with a normal posture. This technique is elegant and employs basic skills learned in plastic surgery residency.

I present a literature search and clinical summaries of patients with calcaneovalgus posture of hindfoot and ankle following burn scar contractures. These patients were treated with posteior capsulotomy and Z-plasty lengthening of skin and Achelles tendon as one unit. This was followed by split thickness skin grafting at the same operation.

These patients were able to start rehabilitation, out of cast at six weeks post-op. The rehabilitation process and prosthesis are reviewed.
