A goal in abdominoplasty surgery is the creation of an aesthetically pleasing umbilicus. Here, we present a modification of a technique that addresses issues of depth, scar position and shape, creating a youthful appearing umbilicus with superior hooding. Prior to the completion of the abdominoplasty closure, after the flap is secured in the midline, the new position of the umbilical opening is marked 1-2 cm below the origin of the stalk. This provides a gentle traction on the new umbilicus. A superiorly based “V” shaped flap is created and defatted, taking care not to devitalize it. A core of subcutaneous fat is removed. A corresponding “V” is excised from the superior portion of the stalk, and inset into the new umbilical opening. The skin flap will fold inwards and upwards to create a superior hood, hiding the superior portion of the umbilical scar. We present 7 consecutive patients on which this technique has been performed. It has been shown to consistently create a pleasing, youthful umbilical shape with a less obvious surgical appearance.
