Reduction mammaplasty is a commonly performed procedure. Many techniques exist to reduce the volume of large breasts while maintaining as aesthetically pleasing appearance. The majority of the commonly used procedures involve variations in the pedicle development. We propose a technical modification of the inferior pedicle technique to improve postoperative aesthetic results. This modification consists of preservation of the native attachments between the lateral border of the pectoralis major muscle to the overlying breast tissue and skin. This septum aids in defining a pocket for the inferior breast pedicle to lie within and prevents lateral displacement. The septum also maintains and further defines the naturally occurring lateral point of inflection between the convex breast mound and concave axilla. The study period from was June 1, 2003 to November 30, 2003 and included ten patients. Preoperative, intraoperative and postoperative photos are presented along with anatomical illustrations. We believe that preservation of the lateral septum during reduction mammaplasty is easy to perform, utilizes native anatomy and improves aesthetic results.
