Increasing enthusiasm for ultrasonic assisted technology and improving awareness of diligent fluid management has advanced the safety and efficacy of suction lipectomy. With continued experience, we reviewed our large-volume liposuction cases over the past five years.
A retrospective review was performed on 81 patients who underwent 84 liposuction procedures of at least 5000 cc total aspirate per procedure. All cases utilized UAL and the superwet technique. Data collection included surgical and anesthetic techniques, patient demographics, perioperative photographs, total aspirate volumes, fluid intakes and outputs and all complications. Average follow-up was one year.
Results showed the majority of patients to be women, aged 19-62 years old. 65% of patients were within fifty pounds of their ideal body weight. Total aspirate volumes ranged from 5 – 13.275 liters. Data analysis revealed an average of 99 cc/kg positive fluid balance at the end of the procedure. No serious complications were experienced.
Our series confirms that large-volume liposuction remains a viable option for body contouring. Adjustments in our practice include multistage procedures for extremely large volume cases and use of solid probe ultrasonic lipolysis. These changes, in addition to continued careful anesthetic and surgical technique, have been effective in maintaining quality outcomes with minimal complications.
