Simultaneous Upper and Lower Eyelids Reconstruction by Using First Web Space Free Flap

Aims: Simultaneous loss of both the upper and lower eyelids is a rare, devastating injury. Furthermore, situations associate with total deep facial burn may lead to difficulty in selecting a suitable donor site for reconstruction and this situations may be challengeable. Patient and Method: We present our experience of reconstructive challenge using first web space free flap for reconstruction of both the upper and lower eyelids defects. The patient had near total full layer defect of both the upper and lower eyelids and there was no available local tissue for donor site in face due to severe facial burn. Consequently, first web space free flap was performed for simultaneous reconstruction of the inner and outer layers of the losing upper and lower eyelids. After the surgery, flap debulking procedures were carried out three times. The patient was followed up for 26 months after the surgery. Results: Postoperative result is good except minor problems that include focal symblepharon and sebum formation. Conclusions: This procedure is not an absolute operative method for total reconstruction of upper and lower eyelid defect, but it can be a effective and alternative method for total eyelid reconstruction, especially in patients without available donor site in face.
