Temporal Approach to Brow and Midface Rejuvenation

This is a retrospective review of a novel and unique application to the temporal approach to brow lift. We present an experience using a 3 cm temporal incision to provide both a brow and midface lift, providing a natural blend between the lateral eyelids and cheek. We used a suturing technique along the deep temporal fascia to provide malar, periorbital, and temporal rejuvenation. We reviewed 106 procedures performed by one surgeon over a 6 year period. We evaluated the age, gender, complications, and outcome of this temporal brow and midface lift, and conclude that it is a safe, reliable procedure that can produce excellent long term results without violating the subperiosteal plane or using a facelift incision. We had no permanent frontal branch injuries, and only 6 (5.7%)transient frontal branch injuries that resolved by 8 weeks postoperatively. Mild alopecia occurred in 3.8%, along with asymmetry requiring revision in 3.8%. We feel that by combining the midface and temporal brow lift through a small hairline incision, patients can experience excellent lateral facial tightening, shortening of lower lid height, an increased bizygomatic diameter, and a more youthful angular appearance with a much quicker recovery time.
