Inferior Pedicle Wise Pattern Reduction Mammaplasty with a Dexon Mesh Sling: Can We Prevent Bottoming Out?

Purpose: The inferior pedicle Wise pattern reduction mammaplasty is a safe and reliable technique however over time the breast tends to bottom out. In attempt to maintain long-term breast projection the senior author has modified the inferior pedicle Wise pattern reduction. Methods and Materials: Dexon is a safe, bioabsorbable material that is commonly used in surgical practice. 17 patients underwent inferior pedicle Wise pattern reduction mammaplasty with a Dexon mesh sling positioned to provide support to the inferior pedicle. Patients were followed with serial nipple to inframammary fold measurements and compared to a cohort of patients undergoing inferior pedicle Wise pattern reduction mammaplasty without mesh. Results: The mesh cohort maintained the desired breast shape and nipple to inframammary fold distance over the short-term, however there was no difference by one year. The technique is safe and without significant complications. Post-operative mammograms were suitable for breast cancer surveillance and demonstrated no distortion of the parenchyma or evidence of the mesh. Conclusion: The inferior pedicle Wise pattern reduction mammaplasty with a Dexon mesh sling is a safe technique that may be used to provide additional lower pole support and limit early bottoming out of the breast.
